Medical Home Statement
Roseville Pediatrics joined the Medical Home Initiative in February 2005. Medical Home is indorsed by all providers and staff within our practice and is an integral part of the philosophy at Roseville Pediatrics. The providers and staff include you and your child as the most important members of the health care team whether ill or healthy.
We are a central resource for all of your child’s healthcare needs and will provide information on pediatric specialists, home care, equipment, and supportive services. We also provide educational materials to your family regarding your child’s health, because information can empower you and your child to improve his/her health. We will help coordinate and plan your child’s care and/or set goals for care.

At Roseville Pediatrics, our Medical Home program is continually evolving to enhance the services that we provide to CYSHCN:
Weekly pre-visit contacts to families are made so providers are aware of any patient questions and concerns prior to appointments.
Timely referrals and prior authorizations so that we can efficiently provide outside care for our patients.
A Medical Home bulletin board located in our waiting room serves as a resource guide for families. It features conferences, programs, camps, and community resources that may be beneficial to CYSHCN and their families.
Our Medical Home team consists of our core team plus Parent Partners who help Roseville Pediatrics implement change by developing strategies and offering advice. Parent Partners attend regular meetings at the Roseville office.
In the past, we have hosted focus groups on Downs Syndrome, ex-preemies, autism, and nutrition. We will continue to explore the best outreach plans to meet our patients’ needs.
Lists of community resources are available to all providers and clinical staff to distribute to patients and their families.
Our patient registries currently consist of over 2000 patients and are updated regularly with new children who are identified by our providers as having special healthcare needs including asthma and ADHD.
Parent profiles are available to families who are seeking guidance and/or support from other families who share similar circumstances with their own child/children.
The Medical Home team at Roseville has presented at National NICHQ (National Initiative for Healthcare Quality) conferences in 2006 and 2009.
We are currently NCQA PCMH Level 3 Certified. NCQA is the National Committee for Quality Assurance; PCMH is Patient-Centered Medical Home.
The Medical Home program at Roseville Pediatrics is constantly growing and changing. Going forward, we will continue to assist families in connecting with one another for support and information. We will also expand our lists of community resources and meet with other agencies to become aware of services within our community.
Our providers will continue to help provide seamless medical care for all of our patients. We will continue to promote our MyLGHealth.org medical chart access so that patients can carry and reference medical history with them anytime.
